ORDERING INFORMATION
You can now view and fill out the order form on your browser, or download the file, fill in the information and email the completed file directly back to Lilly.
For the latest version of Adobe Reader click on the button below.
Mac users: Download the file and open in "Acrobat/Adobe Reader" not "Preview".
After filling in the information, attach to an email and send.
Of course, as always, you can print out an order form and enclose it along with payment via regular mail. In either case please be sure to double-check your information to minimize any errors.
